Renewing an Affidavit for the 2018-2019 School Year

Renewing an Affidavit


If you are a current AUSD Family/Student and are needing to renew an Affidavit please make an appointment with Student Services by clicking on the link below.






If you live with someone and your name is not listed on the rental agreement, property tax, or utility bill, you will need to fill out a Shared Residence Family Affidavit to prove Alameda residency. If your student lives with someone other than his/her parent, you will need to fill out a Shared Residence Student Affidavit. All affidavits are subject to review and mandatory home checks required. All affidavits must be renewed yearly. 


You must have the following required documentation with you at the time of enrollment:

*Notarized affidavit (this form is only available to pick up in person at the AUSD District Office)

*Rental agreement or property tax from the person you are living with, making sure there are no stipulations regarding how many tenants, or which tenants, are allowed in a residence. If the rental agreement is with the Alameda Housing Authority, affidavits are NOT accepted unless the student is added to the rental agreement. 

*Current (last 30 days) utility bill from the person you are living with

*Valid CA State Driver's License or ID or Passport of the person you are living with

*Your valid, current CA State Driver's License/ID or Passport

*One of the following, in parent/guardian name, at address listed on affidavit: bank statement; W2 or pay stub; health, rental or homeowner's insurance; other tax document. 

Incomplete Enrollment Drop In Hours

Did you forget a required document during your enrollment appointment? Need to show proof of an immunization? Need to turn in required affidavit documents? This time is NOT for INITIAL ENROLLMENT APPOINTMENTS. If you have not yet met with an enrollment counselor, please log in to Infosnap and schedule an enrollment appointment. For incomplete enrollments, please plan on returning to our office on one of the following dates:



Tuesday:   10th 1-2 pm, 17th 10-11 am & 24th 10-11 am 


Thursday:  5th 2-3 pm, 12th 8:30-10:30 am, 19th 1-2 pm & 26th 9-10 am



(This drop in time is for INCOMPLETE enrollments only)


If you cannot make this time frame, log in to your Infosnap account and cancel your first appointment so that you may make another appointment to complete enrollment.

Remember- your student is NOT ENROLLED until all required documents are turned in and the enrollment counselor tells you that the enrollment is complete.


All Families can begin online enrollment

TK/K families can make appointments for Feb 5th or later. 1st through 12th grade families can make enrollment appointments for March 5th or later

Current AUSD Students/Families

Renewing an Affidavit
If you are a current AUSD Student/Family and are needing to renew an Affidavit please make an appointment with Student Services by calling (510) 337-7086 to schedule an appointment.
Changing an Address
If you are a current AUSD Student and need to change your address you may come to Student Services with the appropriate paperwork  Monday – Thursday, between 8:00 am – 5:00 pm.  No appointment is needed.   To change your address please bring the following documents:
  • Your Photo I.D.
  • The New Lease/Rental Agreement or Property Tax/Mortgage Statement
  • 1 (one) Current (within the last 30-days) Utility Bill (Alameda Municipal Power, PG& E or Water/Garbage) 
Requesting an Intra-District Transfer (A transfer from one AUSD school to another AUSD school)
If you would like to request an Intra-District transfer please click on the link under quick tools at the top of this page. Fill out and submit the form. 
Requesting an Inter-District Transfer (A transfer from another city into AUSD or a transfer out of AUSD into another city)
We will accept Inter-District Transfer requests from June 18, 2018 through June 29, 2018.  Please include the Inter District request from your city of residence, your students grades, attendance and discipline record (if any) with your request. No appointment is needed.  Our office is open Monday - Thursday 8:00 to 5:00.
If you would like to request a transfer to leave the AUSD and attend school in a different school district you may come to Student Services Monday – Thursday, between 8:00 am – 5:00 pm to turn in your application. No appointment is needed.  
Transcript Request
If you would like to request a copy of your transcript please contact the AUSD high school you attended to place your request.  They will process the transcript request for you.
Work Permit
If you would like to request a work permit please contact your high school’s College and Career Coordinator who will prepare the work permit for you. During the summer, please come to the Student Services Department to get the permit.

Enrolling in AUSD School after Attending a Charter School

Romero Act Open Enrollment 2017-2018 Information

The Romero Act allows for parents whose children currently attend Ruby Bridges Elementary School to request transfers for the 2017-18 school year. The school transfer choices are Haight Elementary and Paden Elementary. Please fill out the application completely and submit it. Transfers are based on space availability at each grade level. Families will be contacted after June 1, 2017 regarding approval.

I 20 International Student Information

We are now accepting I 20 International students for the 2018-19 school year. The application period for I 20 students will begin on March 26, 2018. Applications received before 3/26/18 will not be reviewed or considered. If you applied before 3/26/18, please fill out the application linked below to have your student considered for enrollment into the 18-19 school year. 


Please click here  to access this year's application. We only accept high school age applicants for a limit of one school year. We do not accept grammar or middle school students, or students applying for partial year attendance. We will accept applicants as long as we have space available. The Alameda Unified School District accepts 30 International students each year. Applicants will be notified in May about acceptance via email.
